So picture a plumber in Charleston, 58, who has run his own one-truck business for twenty years. His customers are the older homes on the East End and in South Hills, rentals in Kanawha City, and a steady stream of calls up and down the valley from St. Albans to Marmet. Cast iron, galvanized lines, sump pumps after a heavy rain, water heaters in tight basements. His wife, 56, does the billing and had a heart attack two years ago. She sees a cardiologist in Charleston every few months and takes several daily medications. Neither of them is ready to slow down.

You can still get a nationwide PPO in West Virginia

To be clear: you can still get a nationwide PPO in West Virginia, and I'd quote one for any healthy Charleston couple who asked.

For this couple I wouldn't lead with it. A nationwide PPO is sold privately and priced after a health review, and a recent heart attack is exactly what that review is built to find. The marketplace is different. Kanawha County has 37 medical plans for 2026 from 2 insurance companies, 15 PPOs and 22 HMOs, and every one of them must accept her and can't charge more for her history. On price, the lowest-priced silver PPO costs 3.5% less than the lowest-priced silver HMO for a 40-year-old before any credit, and no silver plan in the county is cheaper to buy. So the marketplace PPO is both the safe choice and, among silver plans, the least expensive one.

Why this renewal deserves a real look

Premiums in West Virginia are high and rising. KFF's average benchmark premium for the state rose 16.8% from 2025 to 2026. A couple in their late fifties feels that more than anyone, which is why the credit matters so much here. It's based on the household's estimated net income for the coverage year, which for a plumber is receipts less the truck, fuel, parts and fixtures, tools, licensing and liability insurance.

Don't let the old plan roll over without checking it. Open enrollment for 2027 coverage is November 1 to January 15, and December 15 is the cutoff for a January 1 start.

My wife's cardiologist has to stay in network

This is the whole decision for this family. Start with her cardiologist, the hospital where she was treated, and every prescription with its dose. We check each against the plans, because two silver plans at similar prices can treat a heart medication very differently. A PPO also pays some of the cost if she ever needs a specialist outside the network, at the clinic in Columbus or Pittsburgh a doctor might send her to. An HMO generally would not, outside an emergency.

Our income is close to the line for the credit

For a couple this age, being just inside or just outside the credit range can make a big difference in what you pay. Estimate carefully. If a strong spring of repipes pushes the year higher than you guessed, raise the estimate on HealthCare.gov right away, because reporting less than you make can mean paying some of the credit back when you file. If work slows, lower it.

We're both healthy and the business did well

Picture the same plumber with a healthy wife. If the household earns past the credit range, every Kanawha County plan is full price, and a medically underwritten nationwide PPO is worth pricing. Both of you answer health questions; the insurer accepts you, accepts with one earlier condition carved out, or declines. Healthy applicants often pay less than full marketplace price, with a network that isn't limited to West Virginia.
