The marketplace PPO sold in Hillsborough County is priced so high that most families skip it, and only one company sells it. There's another route. I can set your family up with a private PPO that has a nationwide network, and when everyone is healthy and you're paying full price, it's frequently the best value I can find.
It mostly comes down to two questions: how much does the household make, and does anyone have a health condition. Kids raise the stakes on both. They add to the household size, which changes where the tax credit line falls, and they add doctor visits you want close to home.
Here's what the county's plan file looks like: 173 medical plans from 7 insurance companies, with only 14 PPOs. At age 40 on silver, the marketplace PPO with the lowest premium is still 138% above the lowest-premium silver plan built on another network.
Picture a couple in their mid-thirties off Lithia Pinecrest. He owns a small HVAC company. She left her job when the second baby came and does the company's scheduling. One kid is in kindergarten, the other is two.
What they care about is the pediatrician they like and a children's ER that's close. HCA Florida Brandon Hospital on Oakfield Drive lists a 24/7 children's emergency center and a Level III NICU, so we check whether each plan reaches it.
Then the money. In a lean year for the business, their income may fall inside the credit range, and a marketplace HMO with that hospital in network is the most affordable answer. In a strong year, it may not, and that's when I'd price a private PPO for all four.

For a lot of Brandon households, the tax credit does most of the work. It only applies on the marketplace, and when you qualify, a marketplace plan almost always beats anything else on price. The key is a solid income estimate, since the credit gets reconciled when you file.
One warning for families with a tough year: the credit generally starts at the federal poverty level, and Florida hasn't expanded Medicaid, so landing too low can leave you with very little help.

If a parent has asthma that needs a specialist, a thyroid condition, or anything that needs regular treatment, the marketplace is the safer place to be. Plans there must enroll you whatever your history and can't charge more for it. A private plan can turn you down or leave the condition out. So we'd keep the family on a marketplace plan that holds onto your doctors.
