Put the silver plans side by side inside the county for a 40-year-old with no subsidy, and the least expensive PPO sits 60% above the least expensive HMO. In Wayne the gap is 51%. HMOs usually pay out of network only in an emergency, so the walk-in clinic in Gary for a bad sinus infection generally isn't covered. A nationwide PPO from outside the marketplace, priced after a look at your health, pays toward that kind of visit at a higher share, no referral needed. What any plan pays on the Ontario side is a separate question, and I check it plan by plan.
The premium tax credit turns on your household's income. For someone who owns the truck, HealthCare.gov wants net self-employment income for the coverage year: settlements minus fuel, the truck payment, tires, repairs, plates, insurance on the rig and your other costs. You estimate it when you enroll and update it as soon as rates, a breakdown or a new contract shift it.
Cross-border freight adds a wrinkle. When parts volumes rise or fall with the plants on both sides of the river, your loads follow, so the estimate deserves a second look mid-year.
Once the household earns beyond the credit range, nothing on HealthCare.gov is discounted, and in St. Clair the marketplace PPO costs 60% more than the HMO beside it. Compare a medically underwritten nationwide PPO instead. The insurer reviews a health questionnaire and approves you, declines you, or approves you with one prior condition excluded. With clean answers the premium can come in below the marketplace's full rate, and the network works in Toledo, Fort Wayne or Joliet just as it does at home.
