Here's how truck drivers in Louisiana are saving money on their health insurance this year

You can still get a nationwide PPO in Louisiana, and a driver running I-20 and I-49 out of Shreveport faces the steepest marketplace PPO price in the state. Here's how Northwest Louisiana owner-operators are handling it.

Hi, I'm Sam Jaber. My whole business is health insurance, my office is in Tampa, my license covers Louisiana, and drivers usually reach me from a truck stop or a loading dock.

Take an owner-operator who lives in Bossier City. Most weeks he heads west on I-20 into Texas, toward Dallas and Fort Worth, or east toward Monroe and Jackson. Other loads go down I-49 to Alexandria and Lafayette and onto I-10, and some keep him out on the road for most of a week. His wife and kids see doctors in Shreveport. He sees whoever's open, wherever he's stopped.

You can still get a nationwide PPO in Louisiana

For that driver, the first thing to know: you can still get a nationwide PPO in Louisiana, and I can set one up.

On the marketplace, a PPO in Shreveport-Bossier costs almost twice as much as the alternatives. Caddo and Bossier each offer 15 HMOs, 29 EPOs, 11 POS plans and 8 PPOs for 2026, and for a 40-year-old on silver with no subsidy, the least expensive PPO runs 97% above the least expensive plan of any other type. That's the widest gap of any Louisiana parish. HMOs and EPOs pay out of network only in an emergency, so the walk-in clinic in Abilene that treats a bad cough generally isn't paid. A PPO picks up part of that bill with no referral, and a nationwide PPO, sold off the marketplace and priced after a medical review, is built for exactly that kind of week.

The truck's profit is the number

The premium tax credit turns on household income for the coverage year. When you own the truck, that's net self-employment income, the money left from settlements after fuel, the truck payment, tires, shop bills, permits and rig insurance. Start with an estimate and change it whenever rates or a breakdown change the year.

I'm gone most of the week and nobody's sick

If the household earns past the credit range, HealthCare.gov charges full price on everything, and the PPO nearly doubles it. A medically underwritten nationwide PPO is the comparison that matters. You answer health questions up front, and the insurer accepts, accepts with one prior condition left out, or turns you down. Clean answers are how a healthy driver can pay less than the marketplace's full price, and a clinic visit in Texas or Mississippi gets treated like one at home. We'll also confirm the family's Shreveport doctors are in the network.

I run regional and sleep at home most nights

Turns to Monroe, Texarkana or Alexandria keep you home most nights. If the household also qualifies for the credit, HealthCare.gov is usually the most affordable route, because the credit works nowhere else. A slow year, or a condition I keep in check for the DOT

Cheap freight or a long repair can leave a household thin, and Louisiana has a real floor: the state expanded Medicaid, so adults with income up to 138% of the federal poverty level can qualify. Drivers watching their blood pressure or sugar for the DOT physical, and households caring for a chronically ill family member, belong on HealthCare.gov whatever they earn, since nobody there can be refused or charged more for health. Occupational accident coverage, if your lease requires it, pays only for injuries on the job.
