Iowa truck drivers

Here's how truck drivers in Iowa are saving money on their health insurance this year

You can still get a nationwide PPO in Iowa, and a driver hauling grain, equipment and freight on I-80 needs one, since his county's marketplace doesn't sell it. Here's how western Iowa owner-operators are setting up coverage.

Sam Jaber, licensed health insurance broker

Hi, I'm Sam Jaber. My business is health insurance, Tampa is home base, Iowa is on my license, and drivers mostly reach me from a truck stop or a co-op elevator line.

Take an owner-operator who lives in Council Bluffs, where I-80 and I-29 meet across the river from Omaha. Some loads are grain and feed moving between elevators and processors in western Iowa; others are farm equipment and parts. The long runs go east on I-80 past Des Moines toward Chicago, west across Nebraska, or down I-29 to Kansas City. His family sees doctors in Council Bluffs. When he's sick on the road, he goes wherever is close.

You can still get a nationwide PPO in Iowa

For that driver, lead with this: you can still get a nationwide PPO in Iowa, and I can set one up no matter where you're parked.

He won't find one on HealthCare.gov at home. Pottawattamie County's 2026 marketplace has 35 HMOs and 31 EPOs from 5 insurers, and no PPO; Iowa's marketplace PPOs are sold only in seven northwest counties. HMOs and EPOs usually pay out of network only in an emergency, which means a walk-in clinic in Cheyenne treating a bad cough generally isn't paid. A PPO covers part of that visit with no referral, and the nationwide version is sold privately and priced after a medical review.

What the truck actually clears

The premium tax credit depends on household income for the coverage year. When you own the truck, that's net self-employment income: settlements less fuel, the note, tires, repairs, permits and insurance on the rig. Enroll with an estimate and change it when harvest freight, rates or a breakdown change the year.

Scenario 1 of 3

Long runs, healthy family

If the household earns past the credit range, every Pottawattamie County plan is full price, and none of them is a PPO. Price a medically underwritten nationwide PPO against them. You answer medical questions first; the insurer then offers a policy, offers one with a single past condition left out, or declines. Clean answers often put a healthy driver below the marketplace's list price, and a clinic visit in Wyoming or Illinois counts the same as one at home. Your family's doctors in Council Bluffs get checked as well.

Scenario 2 of 3

Short hauls, home most nights

Elevator and processor runs around western Iowa keep you home most nights. If the household also falls in the credit range, HealthCare.gov generally comes out ahead on cost, because the credit is only good there. With 66 HMOs and EPOs to pick from, choose the network your family already uses, keep routine visits for home days, and remember that emergencies are covered anywhere.

Scenario 3 of 3

A rough year, or a condition I manage for the DOT

A blown engine, a weak harvest season or soft rates can leave a household thin. Iowa has a floor in the Iowa Health and Wellness Plan, its Medicaid expansion, with an adult income limit of 138% of the federal poverty level. If you keep blood pressure or blood sugar under control for your medical card, or someone at home has a chronic illness, stay on HealthCare.gov whatever the truck earns; its plans can't refuse you or price your health. An occupational accident policy covers work injuries, not sickness.

Where the Iowa figures come from

  • Iowa's individual marketplace runs on HealthCare.gov; Iowa is one of the states in the federal 2026 QHP Landscape file Source
  • For 2026 Iowa's marketplace has 121 individual medical plans from 6 insurers: 71 EPOs, 38 HMOs and 12 PPOs; one insurer offers the PPOs, sold only in Clay, Dickinson, Emmet, Lyon, O'Brien, Osceola and Sioux counties Source
  • For 2026, Pottawattamie County has 35 marketplace HMOs and 31 EPOs from 5 insurers, and no PPO Source
  • HMO plans generally don't cover care outside their network except in an emergency, EPO plans cover out-of-network care only in an emergency, and PPO plans cover out-of-network care at a higher cost without a referral Source
  • Self-employed people report net self-employment income to the marketplace, estimate it for the coverage year, and should update it as soon as it changes Source
  • Iowa has adopted the Medicaid expansion through the Iowa Health and Wellness Plan; adults qualify with income up to 138% of the federal poverty level Source

Questions truck drivers ask me

Straight answers, no sales pitch.

Can I buy a PPO on the marketplace in Council Bluffs?

No. For 2026, Pottawattamie County's marketplace has HMOs and EPOs only. A healthy driver can still get a nationwide PPO privately.

Will an Iowa EPO cover me in another state?

Usually only in an emergency, unless the provider belongs to the plan's network.

Can an Iowa owner-operator get Medicaid after a bad year?

Possibly. Iowa's Medicaid expansion covers adults with income up to 138% of the federal poverty level.

Does every Iowa driver need a nationwide PPO?

No. A driver who's home most nights and qualifies for the credit often does better with a local HMO or EPO.
