Here's how realtors in North Carolina are saving money on their health insurance this year

You can still get a nationwide PPO in North Carolina, through me, whatever the marketplace charges for its own. Here is how agents on the southern coast are choosing coverage around closings, health and the move that brought them here.

Picture an agent who lives in Leland and sells from Wrightsville Beach down to Southport and up to Hampstead. A good share of her buyers are relocating from the Northeast or the Midwest. A few years back she was one of them: she moved here, earned her North Carolina license, and now gets paid at closing like everyone else in the office. No brokerage plan is coming. She buys her own coverage through HealthCare.gov, and on the coast that means a decision most agents in other parts of the country never face. There is a PPO on the shelf, and it costs more.

You can still get a nationwide PPO in North Carolina

In New Hanover, Brunswick and Pender counties, four insurers sell 65 individual medical plans for 2026. Forty-seven are HMOs, nine are POS plans and nine are PPOs. So the coastal agent does have a marketplace PPO to pick from. At age 40 and before any subsidy, its least expensive silver version costs 25% more than the least expensive silver plan of another type.

That isn't the end of the menu. You can still get a nationwide PPO in North Carolina through me, a private plan priced from a health questionnaire, with doctors in network well past the state line. For a healthy agent who has to pay full price either way, that's usually the comparison worth making.

The number that decides your path

The marketplace credit follows your household income for the coverage year. For an agent, that's commission after the split, desk and franchise fees, MLS and board dues, photography, staging and the miles between Leland and the beach towns. It reaches up to four times the federal poverty level, and at the bottom North Carolina's Medicaid expansion, in effect since December 2023, covers adults up to 138% of that same poverty line.

Coastal markets have a rhythm. Listings and showings pile up in spring and summer, and winter can go quiet. Build your estimate on a full year, not a hot June or a slow January.

My year lands in the credit range

Then HealthCare.gov is your store, because the credit can't be spent anywhere else. Here's the part that matters on the coast: the credit is sized from a benchmark silver plan, not from the plan you choose. If you pick the marketplace PPO, the 25% difference comes out of your pocket.

That can still be a fine trade. If your doctors are in its network and the extra monthly cost fits your budget, a credit-backed marketplace PPO is a sound buy, and I'll say so. If the HMO keeps your doctors, the savings are yours to keep.

I closed a big year and I'm in good health

Above the credit range, the full sticker applies, and on the coast the PPO's sticker is the highest on the shelf. This is where many producing agents look at private coverage instead.

You answer health questions, and the insurer approves you, approves you with a named condition excluded, or declines. When you're healthy, that often brings the monthly premium under the marketplace's full rate, and the network is national, which helps if you split time with family out of state or travel for conferences. We read each plan's benefits together before you choose.

I just moved here and got my license

Plenty of agents on this coast arrived from somewhere else. A permanent move to a new state can open a special enrollment window, generally if you had qualifying coverage in the 60 days before you moved, so you don't have to wait for fall. Your first year in a new market is also the hardest to forecast, so we start conservative and update as closings land.

Someone on my plan has an ongoing condition

Stay on the marketplace, even if your income is past the credit range. A marketplace plan accepts you and can't charge more because of a diagnosis. The work is choosing the network that keeps your specialists in Wilmington, and checking whether the PPO's higher price buys you a doctor the HMOs don't have.

When to make the change

Open enrollment for 2027 coverage runs November 1, 2026 to January 15, 2027, and December 15 is the cutoff for a January 1 start. If you're weighing a private plan, start a few weeks before you want it to begin, since approval takes time.
