Here's how realtors in Iowa are saving money on their health insurance this year

You can still get a nationwide PPO in Iowa through me, even though the Des Moines marketplace sells none. Here is how Polk County agents line up coverage with their closings and their health.

Imagine an agent in Ankeny who sells across the Des Moines metro: starter homes in Altoona, new builds in Waukee and Grimes, older bungalows in Beaverdale. Many of her buyers are moving in for insurance and finance jobs downtown, some from Chicago or the Twin Cities. Spring is a sprint, and winter can feel endless. There's no group plan at her brokerage, which leaves her shopping the federal marketplace each fall. In Polk County, she won't find a single PPO there.

You can still get a nationwide PPO in Iowa

Polk County has 58 individual medical plans for 2026: 32 EPOs and 26 HMOs. Statewide, Iowa's 12 marketplace PPOs are all one company's, offered in just 7 of 99 counties, every one of them in the northwest corner. Central Iowa isn't among them.

Through me, you can still get a nationwide PPO in Iowa outside the marketplace. Underwriting sets the price, and the network spans the whole country, which matters for an agent who travels for conferences, family or a winter break.

Commission, the credit and the Medicaid line

HealthCare.gov sets your credit from household income for the coverage year, and help stops above four times the federal poverty level. For a real estate agent, that number is what's left of gross commission after the brokerage takes its share and you've paid for board dues, lockbox and MLS access, advertising, open-house costs and driving.

Iowa expanded Medicaid through the Iowa Health and Wellness Plan, and KFF lists the adult limit at 138% of the poverty level. Iowa is starting a Medicaid work requirement early: KFF's tracker lists December 1, 2026 as the start date.

My income leaves room for the credit

Credit dollars work only on marketplace plans. In Polk County that means an HMO or an EPO, and with either, care outside the network is generally covered just in emergencies. So the decision is about networks: which plan includes your doctors and the Des Moines hospital system you'd use.

My closings were strong and I'm healthy

Once your income passes the credit line, you pay the full sticker for a plan whose network is built around central Iowa. That's when many producing agents price a private nationwide PPO. The underwriter reviews a medical questionnaire and accepts you, accepts you with a named condition excluded, or declines. Agents in good health often land below the marketplace's full price, with doctors in other states counted as in network. We compare benefits plan by plan.

A slow year dropped my income

A stalled spring market can drag an agent's income down fast. Before you enroll, figure out whether the year lands near the poverty line. At or under 138%, the Iowa Health and Wellness Plan may apply, along with its new work requirement. When deals pick back up, update HealthCare.gov and move to a marketplace plan.

Someone on my plan needs ongoing care

A family member with a chronic condition belongs on a marketplace plan whatever you earn. Those plans take every applicant, and health can't raise the price. In the Des Moines metro, the work is matching specialists to an HMO or EPO network.

Enrollment dates

For 2027 plans, HealthCare.gov's window is November 1, 2026 through January 15, 2027; choose by December 15 and your coverage starts January 1. Underwritten plans need time for approval, so begin that comparison in October.
