A general contractor lives in Papillion and builds custom homes in the new subdivisions spreading south and west of Omaha, plus a basement finish or a deck whenever a framing crew is waiting on inspections. You won't find one on the marketplace where you live. Sarpy County's 2026 lineup is 81 medical plans from 5 insurance companies, and all 81 are EPOs. Nebraska does have 9 marketplace PPOs, but one company sells them, in 34 of the state's 93 counties, mostly out west, and the Omaha metro isn't one of them. An EPO pays only for its own network unless it's an emergency. So the urgent care visit for a rolled ankle at a Des Moines tournament, or a sinus infection treated in Breckenridge, is on you unless that clinic happens to be in the plan's network. A nationwide PPO is sold privately, priced after the insurer reviews your health, and is built for a family that moves around like that.

The question that sorts it out

Everything turns on the household's income for the coverage year. For a contractor, HealthCare.gov wants the company's net, not the contract totals: what's left after subs, lumber and concrete, equipment notes, the builder's risk policy and the trucks. You estimate it for the year ahead and change it if a project stalls or a big one lands.

Open enrollment for 2027 coverage starts November 1. Sign up by December 15 if you want the plan in force on January 1; the window closes January 15. That's usually the quiet stretch after the last foundations go in before the ground freezes, which makes it a good time to call.

The company netted well and we're all healthy

This is the family I described. Once the household's income is above the range where the marketplace's credit applies, every one of those 81 Sarpy County EPOs costs full price, and the credit no longer favors HealthCare.gov. That's when a medically underwritten nationwide PPO deserves a real quote.

Underwriting means each person answers health questions up front. The insurer can approve the family, approve it with one past condition left out, or decline. When everyone is healthy, that screening is often what lets the premium come in under the marketplace's full price, and the network travels with you to Kansas City, Des Moines or a ski town. A slow year puts us back in credit range

Building runs in cycles. If rates climb, a few clients walk away from their lots and the year's net shrinks, the household can drop back to where the credit applies. Then HealthCare.gov is usually where you'll pay least, because the credit can only be used on a marketplace plan.
