Here's how 1099 contractors in Louisiana are saving money on their health insurance this year

Shreveport and Bossier City sit close to two state lines, and a lot of local contract work ignores both. Picture a pipeline and facilities inspector who lives in Bossier City. This month his jobs are a compressor station outside Longview, Texas, a plant expansion near Texarkana, Arkansas, and a couple of days on the Louisiana side near Haughton. Every operator pays him on a 1099, none of them offers benefits, and he shops on HealthCare.gov as a Bossier Parish resident. The PPO he finds there costs nearly twice what the HMO does.

Caddo and Bossier parishes each have 63 individual medical plans for 2026: 29 EPOs, 15 HMOs, 11 POS plans and 8 PPOs. For a 40-year-old shopping silver with no subsidy, the PPO costs 97% more than the most affordable HMO. That's the steepest gap of any of Louisiana's 64 parishes; the statewide median is 48%.

When the job is in Texas

HMOs and EPOs generally pay outside their networks only for emergencies. A PPO keeps paying out of network, with a larger share left to you. So the first question for a contractor whose week crosses two state lines is whether the plan's network actually includes the urgent care near the job site, your regular doctor and the hospital you'd pick. A low premium on a plan that won't pay in Longview isn't much of a bargain.

Getting the number right

What HealthCare.gov counts is your self-employment profit for the year you're buying coverage: 1099 pay less your truck, fuel, per diem you aren't reimbursed for, certifications, inspection tools and insurance. When an operator adds or cancels a project, update your estimate soon after.

Louisiana expanded Medicaid in 2016, and KFF lists the adult limit at 138% of the poverty level. Months when you're eligible for Medicaid generally don't come with a marketplace credit. Many adults covered through the expansion will also need to meet a federal work or community-engagement rule, which begins by January 1, 2027.

My net income qualifies for the credit

Shop on HealthCare.gov, since the credit only applies there. The credit is tied to a benchmark silver plan and won't stretch to cover the PPO's near-double price, so start with the HMOs, EPOs and POS plans and check each network against your Texas and Arkansas stops. My income is too high for the credit, and I'm healthy

Paying full price, you'd be picking between networks that may stop at the state line and a PPO priced nearly double. Many healthy contractors do better privately. The application asks about your medical history; the insurer accepts you, accepts you minus one specific condition, or declines. In good health, the premium often comes in below the marketplace's full rate. Plans spell out their own benefits, and we go through them together.

Projects dried up for a while

If an operator pauses work and the year falls toward the Medicaid line, check before you enroll. At or under 138% of the poverty level, Louisiana Medicaid may fit, subject to the coming work requirement. Once inspections pick up again, update HealthCare.gov and switch back to one of its plans.

If a daily prescription or a regular specialist is part of your life, the marketplace is the safer place to be, whatever you earn. Its plans can't turn you away, and your health can't push up the price. Choose the network that includes your specialist, whether they practice in Shreveport or across the line.
