• Get the legal insurer name, exact product name, policy or certificate, benefit schedule and applicable riders. An illustration is not the contract.
• Ask whether the product is ACA-compliant major medical, short-term coverage, fixed indemnity, another supplemental product or a combination. Review each component separately.
• Ask for the Summary of Benefits and Coverage where applicable. If a product does not provide an SBC, request its full benefits and exclusions documents.
• Confirm what requires underwriting, whether an application can be declined or changed, and when acceptance and coverage become effective.

• Read pre-existing-condition language, exclusions, waiting periods and any benefit-specific restrictions.
• Find the deductible, copays and coinsurance. Identify separate limits for prescriptions, outpatient services or other benefits.
• Ask whether the plan has a covered in-network out-of-pocket maximum and what does not count toward it. A fixed benefit payment is not a promise to pay the rest of a medical bill.
• Review per-service, daily, annual and lifetime benefit limits where present. Do not treat a benefit cap as a limit on what you may owe.
• Check renewability, termination provisions, rate-change terms and any cancellation or refund conditions in writing.

• Check your doctors, facilities and prescriptions against the exact plan and network.
• Compare maternity, mental health, preventive care, emergency care and other benefits you need using the written terms. Do not assume coverage.
• Ask how out-of-network charges, authorization and claims are handled.
• Compare against available employer or Marketplace options, including any savings and coverage differences.
• Keep copies of the documents, application answers and written explanations. Do not cancel current coverage before the replacement is confirmed.
