A clear handoff helps an employee compare options without guessing when the employer plan ends. Use this alongside the employer’s required notices and procedures.

Confirm with the benefits team

The last covered day for medical, dental and vision, if offered. Do not substitute the employee’s last workday.
Which dependents are enrolled and affected, confirmed through the employer’s authorized process.
The plan administrator or continuation vendor, official contact information and where questions should be directed.
When and how the employee should expect the continuation notice, if applicable.
Whether an employer contribution or severance subsidy continues, its amount, and its end date in writing.

Provide or identify the documents

Current plan and network names, Summary of Benefits and Coverage, and the insurer member-services contact.
The applicable continuation/election notice and its actual cost, payment and coverage instructions.
Written coverage-end confirmation the employee can use when another coverage source requests evidence.
Instructions for obtaining claims records or deductible information through the insurer’s secure member portal.
A neutral resource link the employee can use privately; do not forward medical histories or dependent details to a broker without authorization.
