Here's how heating and cooling technicians in West Virginia are saving money on their health insurance this year

You can still get a nationwide PPO in West Virginia. For a heating and cooling shop in the southern coalfields after a slow year, though, the first questions are the credit and Medicaid. Here's how Beckley-area techs work it out.

Hi, I'm Sam Jaber. I'm a health insurance broker working from Tampa with a West Virginia license, and I do it all by phone, which suits a tech who spends half the day on two-lane roads.

Here's who I'm picturing. A heating and cooling owner in Beckley with one van and a part-time helper. His customers stretch from town out to Shady Spring, Sophia and the hollows toward Mullens: heat pumps in newer houses, oil and propane furnaces in older ones, and a lot of retirees who need a repair but can't afford a replacement. This year was slow. A mild winter, a cool summer and a couple of big accounts that went elsewhere cut his income hard. He and his wife are in their forties. Their family doctor is in Beckley, but any specialist means an hour or more over mountain roads to Charleston, or longer to Morgantown.

You can still get a nationwide PPO in West Virginia

Here's the short version: a nationwide PPO is still available to you in West Virginia, and I can price one for your household in Beckley.

In a lean year, though, the marketplace and Medicaid come first. Two insurance companies sell Raleigh County's 37 marketplace medical plans for 2026, and 15 of them are PPOs while 22 are HMOs. Price them for one 40-year-old on silver, with no credit yet, and the lowest PPO sits 3.5% below the lowest HMO. That matters for a family who drives to Charleston for specialists: the PPO costs less among silver plans and still pays part of an out-of-network visit, where an HMO generally pays only for emergencies outside its network.

Where a thin year puts you

The credit and Medicaid both turn on the household's income, which for a shop owner is net: receipts minus the helper's pay, the van, fuel, parts, refrigerant, licensing and insurance. Estimate it honestly for the coverage year.

Our income dropped low enough for Medicaid

If the year leaves the household at or under the Medicaid line, that may be your coverage. Know what's changing. Beginning January 1, 2027, certain adults 19 through 64 in West Virginia's Medicaid Adult Expansion Group have to complete 80 hours a month of qualifying activity, like employment, a work program, education or volunteering, to keep coverage. The state also says applicants may have to meet it in the month before they apply, so someone applying for January 2027 may need December 2026 to count. Employment is one of the listed activities, so keep a record of the jobs and hours you work.

We're above the Medicaid line, but not by much

This is where many small shops land in a slow year, and the credit does the heavy lifting. The marketplace is usually your least expensive route, since the credit applies only there. Since Raleigh County's lowest-priced silver PPO undercuts all of its silver HMOs, that's where I'd start, once we confirm the family doctor in Beckley and the specialists you drive to are in its network. If business picks up later in the year, raise your estimate so you don't owe credit back at tax time.

Business came back and we're healthy

A cold winter can turn things around fast. If the household's income climbs past the credit range, you'll pay sticker price for any Raleigh County plan, and it's time to get a quote on a medically underwritten nationwide PPO. Each of you answers health questions; the insurer approves, approves with one past condition excluded, or declines. Healthy people often pay less this way than full marketplace price. If either of you has a condition that needs steady care, stay on the marketplace instead, where no one is turned away or charged more for health.
