You can still get a nationwide PPO in North Carolina, and on parts of the coast the marketplace PPO is close enough in price to deserve a real look. Here's how coastal heating and cooling owners are choosing.

Here's the picture. You own a small heating and cooling company in Morehead City. Summers on the Crystal Coast are thick and humid, heat pumps run hard, and a good share of your work is in rentals and second homes close to the water. When a hurricane comes through, the season changes overnight: flooded outdoor units, soaked ductwork and a replacement list that can keep you booked for months. That makes your income hard to call, and it also makes the coast an unusual place to buy health insurance.
First, the part that holds everywhere: you can still get a nationwide PPO in North Carolina, through me, whenever that's the right fit.
On this stretch of the coast, though, the marketplace PPO is a genuine option, and I'll tell you so. In Carteret, Craven and Dare, the 2026 marketplace lists 19 HMOs and 9 PPOs from 2 insurers. Comparing silver plans in the same county for a 40-year-old before any subsidy, the lowest-priced PPO costs only 3% more than the lowest-priced HMO. Down in New Hanover, measured the same way, the gap is 25%. An HMO generally pays out of network only for emergencies, while a PPO also pays for doctors outside its network, at a higher cost to you and without referrals.
Your premium tax credit rests on your household's net self-employment income for the year: revenue minus equipment, refrigerant, the vans, your license and your insurance. HealthCare.gov takes your estimate when you enroll and wants it updated as soon as it changes.
A storm season can push a coastal HVAC company's year well past the guess you made in the fall. When that happens, raise the estimate right away so the credit adjusts for the months that are left. A quiet year works the other way, and lowering the estimate can raise the credit.
This is where the coast differs from most of North Carolina. Since the PPO here costs barely more than the HMO before credits, a household that qualifies for the credit can often keep out-of-network coverage for very little extra each month. If your family sees doctors at different practices up and down the coast, or you simply want room to choose, I'd look at it seriously. If everyone you see is local and in the HMO's network, the HMO is still a sound pick.

A shop with steady service agreements and a few vans can carry a household beyond the credit range. Then every marketplace plan is full price, the PPO included. For a healthy family, the other lane is a medically underwritten nationwide PPO. The insurer reviews a health questionnaire, then offers coverage, declines, or offers coverage that leaves out one condition from your past. A clean history is how the premium can come in below the marketplace's full rate, and the network isn't tied to the coast. Benefits are set plan by plan, so we read them together.

If a family member manages a chronic condition or takes a daily medication, stay on HealthCare.gov whatever your income. Every marketplace plan accepts you and can't charge more for a health history. On the Crystal Coast, the small PPO gap means you may be able to keep out-of-network coverage for that care without paying much more.
